Privacy and Spark Therapies.

Spark Therapies, LLC is required by a federal law known as the Health Insurance Portability and Accountability Act of 1996 (HIPAA; Pub.L. 104–191, 110 Stat. 1936, enacted August 21, 1996) to keep all health information confidential. Read on to learn about how Spark Therapies works to maintain your family’s privacy and keep your information secure.

Privacy in the Office.

During the initial assessment, Spark Therapies retains a copy of the evaluation in a locked file which is considered part of your child or loved one’s health information.

Once the treatment begins, our records will include a plan of treatment, the subjective-objective and assessment information for individual therapy sessions (aka S.O.A.P. notes), a person-specific orientation, contact information, and any applicable re-evaluation or discharge reports.

Information in this record may be shared as needed between specialists at Spark only when it is deemed to benefit and improve the quality and continuity of care.

A chart is kept with up-to-date information on all progress as part of the services that Spark Therapies provides. State and federal laws exist to protect the privacy of this information, this is called PHI or Protected Health Information.

None of the above-mentioned files or information will be given to anyone without the prior written consent of the client or client’s legal guardian with exception to the legal situations below:

Spark Therapies may disclose health information without your consent or authorization required by law to the following entities or types of entities that include, but are not limited to:

  • The federal Food and Drug Administration
  • Public health or legal authorities charged with disease prevention
  • Correctional institutions
  • Law enforcement, only as required by law or with a valid court-issued subpoena

Disclosures and Uses of Protected Health Information.


Our Specialists will document in the client’s record his or her assessment results as well as regular therapy notes.

Related reports from other providers (which we have obtained through your permission) will also be retained as a part of the file.

Spark Therapies will provide your physicians, or other related healthcare providers, with copies of various reports that should assist him or her in treating your child.

Payment of Services:

Spark Therapies will use your child’s health information to receive payment for services from Medicaid or insurance. To view current fees, see our Costs page.

Disclosures to Others Involved with Therapy and Support.

Spark Therapies may disclose with a family member, other relatives, close personal friend or any other person your identity, any health information that is directly relevant to that person’s involvement in, or payment related to, your child or loved one’s care.

This disclosure will only be acted upon if you agree, via prior written consent.

You have the right to request a restriction on the above uses and disclosures of your protected health information (PHI) for treatment, payment and health care operations; however, we are not required to agree to your request.
Spark Therapies reserves the right to change this Privacy Policy at any time.
Families will receive any updated copies of our Privacy Policy in the event that changes are made and/or notice of these changes as needed and where to access them.
